When a child's fever needs a doctor

Fever is a response, not a disease. How a child behaves between temperature spikes tells you more than the reading itself.

Bring the child in if

Any of the following warrants a same-day paediatric review rather than watchful waiting at home.

  • Temperature above 39°C that does not respond to paracetamol
  • Fever lasting more than three days
  • Rash that does not fade under pressure
  • Fast or laboured breathing, or chest indrawing
  • Fewer wet nappies, dry mouth, no tears when crying
  • Any fever in an infant under three months

What helps at home

Weight-based paracetamol, light clothing, and frequent small sips of fluid. Sponging with cold water and alcohol rubs are not recommended.

What to note before the visit

Time and value of each temperature, doses given, fluid intake and urine output. Two lines on your phone save ten minutes in the consultation room.
